CURRENT STUDENTS will be given the Intent To Return Forms on Tuesday, January 25, 2022.  Please look in their backpacks and return these forms by FRIDAY JANUARY 28, 2022.
 
If MHS is your home school and you want to continue here, please indicate this.  If MHS is not your home school, overflow family, please indicate if you want to continue at MHS or if you prefer to move to your home school.
 
FOR NEW FAMILIES - PLEASE DO NOT REGISTER ONLINE UNTIL YOU PICK UP A NEW FAMILY REGISTRATION PACKET.  These packets can not be picked up and appointments CAN NOT BE SCHEDULED UNTIL FEBRUARY 16.  No appointments will be made before February 16.  The packets will be in a box and appointment slots will be on a clipboard outside the office beginning February 16 and not before.
 
 
 
VERIFICATION OF NEW ADDRESS:
 
If you move, please provide Mrs. Soto with the following 3 proofs of South Pasadena residency verification documents and photo ID of parent/guardian:
 
You need one document from EACH category
 
Category A
Deed of Trust
Mortgage Payment Receipt – dated within the past month
Rental/Lease Agreement, listing all names of occupants (including children), and the name, address, telephone number, and signature of landlord/management company.
Property Tax Bill – dated within the past year
Closing Escrow Papers
 
Category B
Utility Bill (Example: Southern California Edison, The Gas Company, Water Bill) – dated within the past month, all pages of the bill must be provided
Cable/Internet Provider Bill (Example: Time Warner, Direct TV)  – dated within the past month, all pages of the bill must be provided
 
Category C
An Additional Utility Bill [different than what is provided in CATEGORY B] (Example: Southern California Edison, The Gas Company, Water Bill) – dated within the past month, with all pages of bill provided
Current Car Registration – dated within the past year
Car Insurance Statement – dated within the past 6 months to 1 year
Health Insurance Statement – dated within the past month
Bank Statement (or signed letter on bank letterhead with the South Pasadena address) – dated within the past month
Current Federal or State Tax Correspondences - dated within the past 6 months
 
 
All residency documents presented must be original.  No photocopies will be accepted.  Online statements of utility bills will be accepted as long as the name, account #,  address and statement date is within 30 days
